In 1969, Hunter Patch Adams is admitted to Fairfax Hospital Psychiatric due to his unstable and unpredictable personality. When he feels he can no longer stay there, he signs himself out against medical advice. Two years later, as a student, he attends medical school at the University of Virginia. He soon stands out for his extroverted and direct manner: he attends a butchers' convention and is mistaken for one of them. He organizes a reception of questionable taste for a group of gynecologists visiting the faculty and insists on visiting patients before the third year required by the curriculum. Expelled by the dean of the faculty and readmitted by the president, he launches the idea of creating a completely free "hospital of joy," involving some fellow students, Truman and especially Carin, with whom he falls in love. In an old, isolated building, the hospital is born, which soon welcomes many patients. One day, Larry arrives, introduces himself, leaves, and shortly after asks for help via voicemail. Carin goes to Larry's house. The next day, the president summons Patch: Carin has been killed by Larry. Overwhelmed with grief, Patch is about to give everything up, but then regains his confidence. He returns to the faculty, and is once again expelled for his behavior. This time he must face the judgment of the medical board: he delivers a passionate defense of his way of understanding the medical profession and the board acquits him. Finally, graduation day arrives: Patch receives his diploma in the traditional graduation gown. But underneath, he is completely naked.
